The number of Rochester-area doctors and medical offices earning national recognition for transforming into medical homes for their patients has grown significantly.
The National Committee for Quality Assurance has now accredited more than 90 local doctors, nurse practitioners and offices as patient-centered medical homes, meaning the staff work in teams to better coordinate patient care, especially for chronic conditions.
Excellus BlueCross BlueShield and MVP Health Care paid for hiring nurse care coordinators and coordinated other help for 21 physicians at seven practices to make changes and get this recognition.
